Scaling removes plaque, calculus, and bacterial deposits from the tooth surface and below the gumline. Root planing refers to thorough cleaning of the root surfaces to remove deposits and contaminated material. Together, these steps reduce the bacterial burden and inflammation associated with periodontitis in areas a routine preventive cleaning is not intended to treat.
Gingivitis is inflammation of the gums without the attachment and bone loss seen in periodontitis, and it can often be reversed with effective professional and home care (see our Gingivitis Treatment page). Scaling and root planing is recommended when the overall periodontal findings indicate periodontitis that requires treatment below the gumline.

The Procedure
- Comfort measures based on your needs — for milder areas, we may use a topical anesthetic gel such as Gingi-Caine to improve comfort without an injection; local anesthetic is available when more complete numbing is appropriate.
- Scaling to remove plaque and tartar above and below the gumline using hand instruments and ultrasonic tools.
- Thorough cleaning of the root surfaces to remove calculus and bacterial deposits below the gumline.
- Treatment may be completed in one or more appointments depending on how many areas require therapy, the extent of periodontal disease, and your comfort needs.
Recovery & Aftercare
Some mild soreness or sensitivity can occur for a few days after treatment, particularly to hot or cold. Most patients return to their normal routine the same day, although it is best to avoid chewing while the mouth is still numb and to follow the aftercare instructions provided for your case. After active periodontal treatment, many patients are placed on periodontal maintenance approximately every three to four months, although the interval is individualized based on disease severity, risk factors, and response to treatment. Periodontitis affects the tissues that support the teeth and can lead to progressive attachment and bone loss if it remains active. Bone and attachment lost to periodontitis do not predictably return after routine scaling and root planing, which is why preventing additional disease progression is an important goal of treatment.
